Comprehensive Medicine is excited to offer a unique and dynamic opportunity for a Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team!
We are a well-established, independent Internal Medicine practice committed to continuity of care across various settings. Based in Alpine, CA—just 15 minutes east of El Cajon—we offer both Primary Care and Urgent Care services , as well as mobile house calls throughout San Diego County.
Position Overview
We are seeking a compassionate, versatile, and skilled provider to work in a hybrid role , with a tailored mix of responsibilities based on experience and interest. The role may include:
Primary & Urgent Care (Alpine Office)
Provide in-office care for scheduled primary care visits and walk-in urgent care cases.
Utilize on-site X-ray, IV therapy, POC labs, suturing, and strong clinical decision-making skills.
Mobile House Calls
Serve as a primary care provider to patients in assisted living facilities, retirement communities, and private homes across San Diego.
Collaborate with third-party providers for in-home diagnostics (labs, imaging, EKGs, etc.).
Work closely with facility staff, home health, and hospice teams to provide coordinated care.
This role offers significant autonomy while maintaining a strong support system from our experienced physicians, mid-level providers, and clinical team.

Clinical confidence in urgent care procedures (suturing, IV therapy, etc.) and primary care management.

Commitment to high-quality documentation using Athena EMR.
Interest in a long-term position with potential for professional growth.
Compensation & Benefits
Competitive salary, based on experience and team fit
6 paid holidays and 80 hours PTO in year one , increasing to 120 hours after one year
$500 annual CME allowance
Mileage reimbursement for house calls
401(k) with employer match after one year

No on-call responsibilities
